Our Mission

Homesura collaborates with communities, nonprofits, and public agencies to develop sustainable, scalable initiatives that enhance care for Medicaid recipients and other underserved groups in Minnesota—making care more coordinated, equitable, culturally appropriate and people-centered.

Our Vision

Homesura's vision is a coordinated network of community partners working together to create a more equitable, people-centered healthcare system—one where everyone, regardless of income, location, or ability, can access coordinated, self-directed care.

Our Values

People-Centered

We put people—not the existing systems—at the center of everything we do. Our work is guided by compassion, equity, and respect for the strengths and voices of those most affected by the Medicaid system. Homesura supports care models that empower people to direct their own services, live independently, and receive care that reflects their culture, community, and dignity.

Our History

In 2025, the non-profit Service Management Group sold Accra, Minnesota’s largest home care agency, to TEAM Services Group, a national home care provider.

In Minnesota, the proceeds of a nonprofit sale must be used to further charitable purposes; thus, Homesura was born.
